Insurance Information
As a concerned provider, we understand the importance of your health care and cost. Please take a moment to review the following information regarding your medical services and insurances accepted.
Every effort has been made to ensure that our charges accurately reflect the complexity of care rendered and the skill and expertise required for your care. As a courtesy to you, we file many forms of insurance including but not limited to: Blue Cross/Blue Shield, Medicare, United HealthCare, Aetna, Cigna, Viva, HealthSpring and PHCS. It is your responsibility to provide accurate insurance information to ensure prompt payment. Ultimately, all patients are responsible for their medical charges. If no payment is received from the insurance company within 60 days of the date of service, payment will be due from the patient.
It is your responsibility to verify with your insurance carrier prior to your appointment that your plan covers the provider and services you have chosen. If your insurance requires a referral from your primary care physician, you must obtain this prior to your appointment.
Medicaid
Our office accepts Medicaid for pregnancy and gynecological visits by referral only. Medicaid patients requesting a visit for a gynecological problem must have the primary care provider listed on their Medicaid eligibility call our office to schedule the appointment in addition to providing a written referral. We do not accept Medicaid for annual exams and our physicians do not participate in Medicaid's Plan First/Family Planning Program with the exception of tubal ligations.
All pregnant patients that plan to use Medicaid for their pregnancy must first enroll in the Alabama Health Network maternity program.

Office Policies
Co-pays
All co-pays are due at the time of service. If you are unsure how much your co-pay is, please contact your insurance company prior to your appointment. If you have insurance and you do not have your correct insurance information with you at the time of your appointment we will have to put you in as self pay until you provide us with your correct insurance information
Self Pay
Patients that do not have insurance are considered self pay. Payment in full is expected at the time of service unless you have discussed otherwise with our insurance coordinator.
Outstanding Balances
Patients with outstanding balances will be expected to take care of this balance at the time of their appointment. Please call and speak with one of our Financial Coordinators at (205) 349-4131 ext. 137 or 138 to set up payment arrangements if you are not able to make full payment of the outstanding balance.
Services Charges
There is a $20 Service Charge for all returned checks.
